Evelyn Constance Wozniak
Evelyn Constance Wozniak (nee Wysocki) of Brookfield died Thursday, June 22, 2006. A native of north Chicago, she graduated from Waukegan High School and St. Therese Nursing School and later in life went on to study psychology at Barrett College.
Upon graduation from nursing school, she served as a second lieutenant in the U.S. Army Nurse Corps during World War II. Evelyn retired as a psychiatric triage nurse from the North Chicago Veterans Affairs Medical Center. She also had worked as an obstetrics nurse at St. Therese Hospital in Waukegan and as a family practice nurse at the Stanul Medical Clinic and with the cardiac unit of the veteranís hospital in North Chicago.
She is survived by a son, Michael (Kathy); a daughter, Barbara (Philip) LeClaire; grandchildren, Jennifer (Charlie) Luizza and John and Philip LeClaire; great-grandchildren, Julianna and Charlie Luizza; brother-in-law, Edwin Gorski; sisters-in-law, Jean Gorski, Adeline Wozniak and Ann Patricelli; and many nieces and nephews.
She was preceded in death by parents, Walter and Helen Wysocki; her husband of 46 years, John M. Wozniak Jr.; and a brother, Sigmund Wysocki.
Chapel services will be held at Ascension Cemetery at 10:30 a.m. Monday, June 26, 2006, with the Rev. James E. Merold officiating. There will be no visitation.
