Dr. Simon Tan, MD, 75, was born to eternal life on Oct. 28, 2009.
He was the beloved husband of the late Ladda Tan. Dear father of George, Peter (special friend, Melissa Mielke), Melissa Ann (Michael) Jarvis and Selena. Loving grandfather of Emilio and Acacia. Uncle of Evelyn (Stephen) Brown. Godfather of Jerome Siy, MD. Dear friend of Lucio, MD and Conchita Siy, MD. Further survived by brothers, sisters, other relatives and many dear friends.
Dr. Tan was a March 1960 graduate of the University of Santo Thomas, College of Medicine and Surgery, Manila, Philippines. He completed an externship at Mary Johnson Hospital, Manila, completed an internship at Deaconness Hospital, Buffalo, N.Y., completed a pediatric residency at Cook County Hospital, Chicago, from 1962 to 1964, was chief resident of the pediatric training program at Little Company of Mary, Evergreen, Ill., from 1964 to 1965, completed his pediatric fellowship in cardiology, hematology and neurology at the University of Chicago from 1965 to 1966. He worked for the Illinois State Institution for Retarded Children from 1966 to 1971, for General Clinic of West Bend from 1971 to 1992 and for Shafi Medical Center in Milwaukee from 1992 to 2009. Dr Tan was awarded a certificate in recognition of his outstanding and distinguished service from Children’s Hospital of Wisconsin in 2003.
Visitation will be on Thursday, Nov. 5, 2009, at St. Jude the Apostle Catholic Church, 734 Glenview Ave., Wauwatosa, from 4 p.m. until the time of memorial Mass at 7 p.m. In lieu of flowers, memorials to the Leukemia & Lymphoma Society are appreciated.
