Dorothy O. Jazak, 87 Dorothy O. Jazak (nee Fritz), 87, passed away Friday, Jan. 8, 2016, at The Kathy Hospice.

Dorothy was born Feb. 2, 1928, in Fond du Lac to Henry and Anita (nee Breuer) Fritz. Dorothy was raised in Allenton and later married the late Victor “Vic” Jazak on June 22, 1946, at Sacred Heart Catholic Church in Allenton. The couple moved to West Bend in 1954 and raised their family. She loved playing the accordion in their polka band, “Vic’s Polkateers.” Dorothy also enjoyed quilting, baking and cake decorating — along with polka dancing and all animals. She was a member of the Addison Quilting Club.

Dorothy is survived by her children, Jerome (Joann), James (Sandra), Sharon (Robert) Smith and John (Susan); and grandchildren, Lisa (Preston) Harris, Michael (Kristen) Jazak, Brian (Melanie) Jazak, Ryan (Karla) Jazak, Eric Jazak, Cory (Kristie) Smith, Kevin (Kerry) Smith, Nicole (Brian) Berres, Kelly Isselman, Heidi (Craig) Remich, and Tyler Jazak. She is further survived by her great-grandchildren, Hayden, Chloey and Brynn Berres, Evan and Blake Harris, Luke and Abigail Jazak, Hannah Jazak, Eric and Colton Jazak, Madison and Ellie Smith, Caleb and Ava Smith, Carsen Isselman, and twins Madelyn and Makayla Remich; sister, Veronica (Leroy) Doll; brother, Eugene (Janet) Fritz; and brothers-in-law, sisters-in-law, nieces, nephews, special friend Al Bartelt, other relatives and many friends. She is preceded in death by her parents; husband, Vic; son, Gerald “Gary”; and sister, Marilyn Steckbauer.

A Mass of Christian Burial will be at 11 a.m. Wednesday, Jan. 13, 2016, at St. Frances Cabrini Church (1025 S. Seventh Ave., West Bend) with the Rev. Enrique Hernandez presiding. Interment will be at Holy Angels Cemetery. Visitation will be 3-6:30 p.m. Tuesday, Jan. 12, at the Phillip Funeral Home Chapel in West Bend with a Prayer Vigil at 6:30 p.m. Continued visitation will be at St. Frances Cabrini on Wednesday, Jan. 13, from 10 a.m. until time of mass.
